My name is Caroline and I am the manager for Peartree community services. I cared for my nan for 7 years after her diagnosis of vascular dementia and Alzheimer’s disease.
I know with every fibre of my being the gut wrenching, heart breaking fear and grief these diseases can cause for a family. GREAT (goal orientated rehabilitation in early stage Alzheimer’s and other dementia’s) is a ground breaking approach using personalised goals via occupational therapy support to significantly improve the lives of people living with mild to moderate dementia. FAQ's
Does my loved one need a diagnosis?
Yes, you need a diagnosis before being treated by us.
Do you do home visits?
You can come to our place, in your own home or in areas important to your loved one.
What if they don’t accept their diagnosis?
Not all people do, that’s ok, but they do need to be able to engage with our therapists. This would be something we would discuss as initial assessments. We would be upfront about it.
What happens at the initial assessment?
One of our occupational therapists will meet with you at the hub or at home and talk to the patient and their family or carer if available and od a series of cognition tests. This would include memory and processing. Work out goals and quality of life goals. Look at your environment to see what tool and techniques can be useful for the client. Information gathering, find out exactly what is important to you.
How personalised is the service?
Absolutely bespoke, it’s about what is most important in life to you and your family.
What dementia can you help with?
This pertains to mild to moderate onset dementia. During the initial assessment, we will review the diagnosis and evaluate the current stage of the condition.
What if my loved one's need change?
We are always honest if your loved one is no longer able to engage with the treatment. We can also adjust the frequency and duration of sessions to accommodate fatigue. Since our charges are one-off, we have the flexibility to modify or reduce the service as needs evolve. This flexibility is one of the key benefits of our service.
Additionally, we provide ongoing support with referrals to other services specializing in more advanced stages of the condition.
What is GREAT cognitive rehabilitation?
The GREAT Cognitive Rehabilitation approach was created by a research team in the UK, at the University of Exter, and was developed based on many years of research. The efficacy of the approach was demonstrated in the large-scale GREAT randomised controlled trial, which involved nearly 500 people with dementia and their care partners. The program has been recommended for use by the National Institute for Health and Care Excellence (NICE) and is promoted across NHS Trusts and other dementia care services. The training and intervention principles are now disseminated through courses and toolkits for NHS and social care professionals.
